Summit County Public Health provides certified birth and death certificate copies for all Ohio counties for every person who was born or died in the State of Ohio from 1909 – present to anyone who can provide the basic facts required to locate the record. Identification and proof of relationship are not required to obtain certified birth and death certificate copies unless you are requesting a certified death certificate and need the record to show the deceased person's Social Security Number within five years of the death.
Summit County Public Health does not have birth or death certificate records for other states or countries. Uncertified birth, death, or fetal death certificate copies are not available per Ohio law.
Summit County Public Health offers several convenient ways to order certified birth and death certificates. Same-day service is available at our Fairway Center location in Akron at 1867 West Market Street, Akron, Ohio 44313.
The cost to purchase a certified birth or death certificate at Summit County Public Health is $22.00 per copy. Cash, checks, money orders, major debit/credit cards are all accepted. To place an order online, complete the order form below (An additional $1.95 credit card processing fee plus fees for mail delivery will apply.)
