⚠In accordance with ORC 3705.23(5) For the first five years after a decedent's death, a decedent's social security number shall not be included on a certified copy of the decedent's death certificate unless that information is specifically requested by the decedent's spouse, or lineal descendant ; the decedent's executor, attorney, or legal agent ; a representative of an investigative government agency; a private investigator; a funeral director (or agent responsible for disposition of the body) acting on behalf of the decedent's family; a veteran’s service officer; or an accredited member of the media .

Individuals requesting a death certificate with the social security number included must indicate on their application that they are requesting the SSN be included and submit proof of identification along with Acceptable Proof of Relationship documentation to the registrar or clerk. The Proof of Relationship Verification Addendum can be used to request that the social security number appears on a death certificate.

Proper identification and proof of relationship is required of in person, curbside, online and mail order requests for a death record.

  9. Notarized affidavit of relationship

BIRTH / DEATH CERTIFICATES AND PERMITS

Online Order Form

Summit County Public Health provides certified birth certificate copies for every person born in the State of Ohio and certified copies of death records for every person who died within Summit County. Checks and money orders should be made payable to Summit County Public Health and mailed to 1867 West Market Street, Akron, Ohio 44313. Credit and debit cards are also accepted in house, over the phone or online (additional fees apply) using the order form below.

1) Pursuant to Ohio Revise Code 3705.29, it is unlawful to purposely obtain, possess, use, sell, furnish, or attempt to obtain, possess, use, sell or furnish to another for the purpose of deception any certificate, record or certified copy of it that relates to the birth of another person, whether living or dead.

2) Refund/Exchange Policy: Summit County Public Health (SCPH) does not provide refunds for Birth and Death Certificate sales. All sales are considered final. If an error is found on the birth certificate copy that is not recorded on the original birth certificate, SCPH will correct and replace the birth certificate within a six-month time period from the date of purchase at no additional charge to the customer.

3) Shipping Policy: SCPH does not guarantee and is not responsible for replacing lost or undelivered certificate requests of USPS First Class Mail.

4) Please note that this website (vitals.scph.org) and www.vitalchek.com are the only websites affiliated with Summit County Public Health. When ordering through any other website you may be bound by their terms and conditions. Summit County Public Health is not responsible for any fees, payments, or refunds of payments made through another website.
